Below is an alphabetical list of some of the most common disability insurance companies we see.  If your policy is through one of the insurance companies listed below, we have experience working with disability claims of this type.   If you have a denial letter from an insurance company listed on this page, or any other disability insurance company, give our office a call to see if we can help!  You will typically have 180 days to submit an appeal on a denied disability claim.  Call us before you do your own appeal.

Aetna is an insurance company based out of Connecticut that offers dental and vision plans, as well as medical plans such as life, disability, long-term care, and health insurance.  For disability, they can be reached at 866-236-4089. Aetna has recently merged with Hartford.

 

CIGNA (LINA) (https://www.cigna.com)

CIGNA, or Life Insurance Company of North America (LINA) is an insurance company based out of Connecticut and Pennsylvania that offers medical, dental, accident, life, and death insurance.  For disability claims, they can be reached at 888-842-4462.

In Melech v. Life Ins. Co. of North America, 739 F,3d 663 (11th Cir. 2014), the Eleventh Circuit Court of Appeals held that procedural fairness required the ERISA administrator to consider evidence from the Social Security Administration process before making a decision under a disability insurance policy.

Guardian is an insurance company based out of Massachusetts that offers Life, Disability, and Dental insurance as well as annuities and investment.  For disability, they can be reached at 888-482-7342.

Hartford is an insurance company based out of Connecticut that offers Car, Home, Business, Life and Accident, and Disability insurance.  For disability, they can be reached at 800-549-6514.

In Menard v. Hartford Life and Acc. Ins. Co., 2008 WL 506228 (11th Cir., Feb 14, 2008), Judge Presnell of the Middle District of Florida held that attorneys fees for the ERISA plan participant were warranted applying the five-step test for awarding ERISA attorneys’ fees.

Libery Mutual is an insurance company based out of Massachusetts that offers a variety of Auto and Property insurance, as well as Life, Accident, Critical Illness, Identity Theft, Pet, Tuition, Small Business, and Workers Compensation.  Small business customers can be reached at 844-525-2467 and mid to large business customers can be reached at 800-362-0000. Liberty Mutual has recently merged with Lincoln Financial.

Lincoln Financial is an insurance company based out of Indiana that offers Life, Disability, Dental, Critical Illness, Accident, and Vision insurance.  For disability, they can be reached at 800-487-1485.

 

MetLife (Metropolitan Life Insurance Company) (https://www.metlife.com/)

MetLife is an insurance company based out of New York that offers Auto, Home, Dental, Life, Disability, Vision, and Accident & Health insurance.  For disability through an employer, they can be reached at 800-858-6506. For disability purchased on your own, they can be reached at 800-929-1492.

In Metropolitan Life Ins. Co. v. Glenn, 554 U.S. 105 (2008), the United States Supreme Court held that a reviewing court should consider the conflict of interest arising from the dual role of an entity as an ERISA administrator and payer of benefits as a factor in determining whether the administrator has abused its discretion in denying benefits.

Prudential is an insurance company based out of New Jersey that offers Disability and Life Insurance, Retirement Pension Plans, Annuities, Mutual Funds and Investments, and Long-Term Care Policies.  For disability, they can be reached at 800-842-1718.

In Calmbacher v. Prudential Ins. Co. of America, 392 F. Supp.2d 1364 (M.D.Fla. 2005), Judge Lazzara of the Middle Distrct of Florida held that the use of the word “determine” did not convey a clear express grant of discretionary authority to the ERISA administrator.

Reliance Standard is an insurance company based out of New York that offers Disability, Life, Accidental Death and Dismemberment, Critical Illness, Voluntary Hospital Indemnity, Group Accident, and Group Dental and Vision Insurance.  For short-term disability, they can be reached at 800-368-2859, and 800-351-7500 for long-term disability.

In Kaviani v. Reliance Standard Life Insurance Company, 799 Fed.Appx. 753 (11th Cir. 2020), the Eleventh Circuit held that the ERISA administrator acted unreasonably when it ignored medical evidence unfavorable for the administrator and selected       medical evidence favorable to the administrator in medical reports to rely on in denying long-term disability benefits.

United of Omaha is an insurance company based out of Omaha, Nebraska that offers medicare supplement , dental, life, long-term care, disability, critical illness, and cancer, heart-attack, and stroke insurance.  For disability, they can be reached at 800-775-1000.

In Horneland v. United of Omaha Insurance Company, 717 Fed.Appx. 846 (11th Cir. 2017) the Eleventh Circuit held that the ERISA administrator was not entitled to summary judgment because the ERISA plan participant’s back pain and muscle spasms were not by themselves necessarily pre-existing conditions.
